Heavy metal is now over 40 years old. It emerged at the tail end of the 1960s in the work of bands including Iron Butterfly, Vanilla Fudge, Jimi Hendrix, Deep Purple, Led Zeppelin and - most importantly - Black Sabbath. In the 1970s and early 1980s, heavy metal crystallised as a genre as bands such as Judas Priest and Iron Maiden removed most of the blues influence on the genre, codifying a set of basic metal characteristics that endure to this day: distorted guitars, aggressive vocals, denim, leather and spikes. In broad terms, wherever it is found and however it is played, metal tends to be dominated by a distinctive commitment to 'transgressive' themes and musicality causing it to be frequently seen as controversial music. Controversies surrounding the alleged (and often documented) connection between heavy metal and, variously, sexual promiscuity, occultism and Satanism, subliminal messages, suicide and violence have all made heavy metal a target of moral panics over popular culture. Metal has variously embraced, rejected, played with and tried to ignore this controversy.At times, the controversy dies down and the previously transgressive becomes relatively harmless - as in the transformation of Ozzy Osbourne from public enemy to loveable dad. Still, metal remains irrevocably marked by its controversial, transgressive tendencies. Indeed, the various moral panics that metal has been subjected to are not only constitutive, at least in part, of metal scenes, but are encoded in metal's transgression itself. As with hiphop's "ghetto" roots, metal's history of extreme sonic, lyrical and visual messages continue to give it credibility with new generations of fans today. The aim of this anthology is to analyse the relationship between heavy metal and society within a global context. It provides a thorough investigation of how and why metal becomes controversial, how metal 'scenes' are formed and examines the relationship between metal and society, including how fans, musicians and the media create the culture of heavy metal.

It is a great pleasure and honor to be asked to participate in the translation of this important and historical volume on thoracic surgery and to provide this foreword. Martin Kirschner of Mannheim/Heidelberg was an early pioneer in thoracic and esophageal surgery. His operation for bypass of the esophagus using the entire stomach remains a standard of the surgical armamentarium to this date. The original Kirschner Textbook of Surgery was a standard in its day. We are fortunate that Professors H. Pichlmaier and F.W. Schildberg and other colleagues have provided us with this important modern successor of a classic textbook. The reader is rewarded by an extensive treatise which includes not only the most up to date techniques in pulmonary, esophageal, mediastinal, and chest wall (including breast) surgery but also provides an excellent perspective on the techniques used by pioneers in the field. Although some of these early techniques are not commonly used today, knowledge of their use and application broadens the capability of the thoracic surgeon. Changing times bring renewed problems with infectious diseases. Knowledge of the management of the pleural space and pulmonary infectious problems is a critical part of the education of the thoracic surgeon.

The original mediaeval manuscripts are unique, being written in Middle Welsh, whereas across England and the rest of Europe, Latin was the universal language for documents. Nicholas Culpeper's famous 17th century medical handbooks are at least five hundred years later than the genesis of the works of the Myddfai mediciners. Around 1000 Welsh remedies and suggestions for a healthy life make fascinating reading, with many being useful today. The evocative Legend of the Lady of the Lake (Llyn y Fan Fach) is included. This tale is the starting point for the line of practising Myddfai physicians from being court mediciners to Prince Rhys Gryg (d.1234) until the present day. Not only has the herbal and healing tradition carried on for around 800 years, but the places named in the legend can still be seen, and many of the plant cures have uses in modern medicine. About the Author Terry Breverton has written over forty books, with his main focus being upon Welsh history, heritage and culture. A former businessman, consultant and academic, he has worked all over the world, has given talks about Wales in Boston, Washington, Vancouver and elsewhere, and has appeared in several documentaries. Part of this book explores the influence of the Tolkien industry, in areas like computer gaming, pop music, merchandizing and cinema. EXTRACT FROM THE INTRODUCTION Philip Toynbee declared, in 1961, that Tolkien's 'childish books had passed into a merciful oblivion', a wonderful statement, just a tad inaccurate. In 1997, The Lord of the Rings was voted the top book of the 20th century by readers in a British bookstore's poll (Waterstone's). 104 out of 105 stores and 25,000 readers put The Lord of the Rings at the top (1984 was second). Around 100 million copies of The Lord of the Rings had been sold by the end of the twentieth century, and 60 million copies of The Hobbit, with sales of around 3 million per year of the two books combined. Readers just love reading Tolkien's books. It's that simple. As fantasy author Terry Brooks said, Tolkien 'was the premier fantasy writer of the last century, and all of us writing today owe him a huge debt.' No other writer W.H. Auden reckoned had 'created an imaginary world and a history in such detail'. Colin Wilson agreed that only a few writers have concocted a total universe, and that Tolkien's was very impressive. Tolkien's mythological writings may be the 'largest body of invented mythology in the history of literature', according to David Day. Invented, that is, by one person. It's also 'certainly the most complex and detailed invented world in all literature'.

The chanson "Doulce Memoire", by Pierre Regnault, 'dit Sandrin' (ca. 1490-1561), is a simple melody elegantly wedded to poetry that evokes nostalgia for something everyone has experienced - a consuming, passionate love that exists only in bittersweet memory. This chanson was so popular that it was frequently reprinted and arranged in a variety of settings for singers and instrumentalists. From 36 known arrangements, George Houle has selected 24 versions that require from one to six performers. Lutenists will find intabulations of varying difficulty, and the keyboard pieces range from the very simple to an elaborate fantasy by Hernando de Cabezon. There are virtuosic arrangements for solo viola da gamba as well as duos and trios for many combinations of voices and instruments. These settings, coupled with Houle's informative text, illustrate the ways "Doulce Memoire" was performed by Renaissance musicians. Some versions show the players conforming to the ideas of the musical theorists of the day, while others demonstrate how much freedom an individual could exercise.
